Health Insurance Health Clearance & Immunization Records Pagers Radiation Hazard Training
All UCLA students are automatically assessed for and enrolled in the University of California Student Health Insurance Plan (UCSHIP) as a condition of registration at UCLA. Continued enrollment in a qualified medical/health insurance plan must be maintained during all registered terms.
The UCSHIP fee is billed each term along with other UCLA fees. UCSHIP fulfills all of the requirements mandated for a qualified medical/health insurance plan as defined by the University. The Ashe Student Health and Wellness Center is the primary healthcare provider for UCSHIP and is where all nonemergency medical care must be initiated for UCSHIP claim payment consideration. Health Clearance & Immunization Records
The David Geffen School of Medicine at UCLA requires that all incoming students obtain a health clearance for personal protection as well as community health. If health clearance is not done at the Arthur Ashe Student Health & Wellness Center, clearance documentation must be sent the Arthur Ashe Student Health & Wellness Center. Students must complete all of the requirements by the first day of class or Pthey may not be allowed to start class.
The health clearance requirements include:
Demonstration of immunization for or immunity to the following:
Tuberculosis testing required annually
How to obtain a health clearance:
Print out the Health Clearance form so you can take it to your health care provider. Once you have completed the Health Clearance requirements, enter them in the electronic version of the form for submission.
Once you are compliant with all of the requirements, print out the page from the Immunizations section and submit it to your Student Affairs Department
Plan A: You may have the health clearance and some or all of the requirements completed at the UCLA – Arthur Ashe Student Health & Wellness Center. Without SHIP insurance, you will be charged $30 for the initial health clearance appointment. You are also responsible for the cost of any lab and immunization fees. Plan B: You may choose to have the health clearance form completed by your health care provider. If you choose to use your own health care provider, be sure to keep copies of your paperwork for yourself and mail or email all documents (form and lab reports) to either:
Keeping Your Ashe Health Record Updated: It is important for all students to keep their immunizations up-to-date and send Student Health Center copies of all immunization/laboratory tests done outside of the Ashe Student Health Center. This applies to all students whether or not they use SHIP for their health insurance. Most hospitals will not allow students to precept/rotate if their Ashe Student Health record is not up-to-date.
Immunization/labs can be faxed, mailed, or e-mailed to the Student Health Center and should include your UCLA ID and the words “Medical Student”.

Radiation Hazard Training
A new Radiation Hazard Awareness Training is now online as part of a Health Systems initiative to ensure our education about radiation hazards in the workplace. This brief training was developed for all RRMC, RNPH and Santa Monica hospital staff to alleviate concerns about working around radioactive materials so that we are able to focus on what we do best every day: providing quality patient care.
Follow this link and click on Radiation Hazard Awareness Training. It will take you less than 10 minutes to complete, and focuses on potential hazards, sources of radiation and signage.
Some employees may be required to complete additional modules, depending on job function. Following the hazard awareness training, go back to the website above and answer the questions designed to identify additional training needed. This will apply to you if you:
