Ramping up onsite, safely

Onsite ramp-up concerns and questions during the COVID-19 pandemic

Consistently and accurately following our COVID-19 safety protocols – daily completion of the UCLA Symptoms Survey, physical distancing, frequent hand washing and wearing face coverings whenever onsite or in public – prevents the spread of infection and saves lives.

Compliance with city and campus guidelines is also critical to our ability to gradually scale up our onsite activities. To report a policy infraction, an incidence of non-compliance or a safety concern, utilize one or more of the below resources.

UCLA WHISTLEBLOWER HOTLINE

  • You can report a concern online or by phone (800-403-4744).
  • Anonymous reporting is an option.
  • Use this resource to report any activity that violates University policy, violates a state for federal law or regulation, wastes money or involves gross misconduct, gross incompetence or gross inefficiency.
  • Reporting of unsafe behaviors is protected under UCLA’s Whistleblower policy.
  • Please do not use this form to report events presenting an immediate threat to life or property. If you experience a life-threatening emergency, call 911. If you require urgent assistance, please contact your supervisor, department head and/or Environmental Health & Safety (310-825-5689).

Infractions, concerns or questions related to onsite ramp-up activities at DGSOM

Examples include concerns over workplace density, masking compliance and physical distancing.

OPTION 1: Contact your supervisor, PI or area leader 

The best place to start with any question or concern is with your immediate supervisor, PI or area leader (department Chair, Division Chief, Chief Administrative Officer, Program Director). These individuals should be the most familiar with your particular questions and should provide a timely response or seek alternative solutions.

Every leader and PI is responsible for ensuring compliance with UCLA and DGSOM COVID-19 policies that have been instated to protect the health and safety of our community and those performing essential functions on campus.

OPTION 2: DGSOM Ramp-up Reporting Form

ACCESS FORM

USE this form if:

  • You are a DGSOM faculty member, trainee or staff member and have questions or concerns related to onsite ramp-up activities at the DGSOM, including general questions or reports of non-compliance with COVID-19 ramp-up guidelines and policies. 
    • Please note: The DGSOM Ramp-up Reporting Form is intended for questions or concerns pertaining to DGSOM research and academic settings. For trainees with issues pertaining to the clinical care environment, questions and concerns should be addressed during huddles, escalated up your clinical chain-of-command and, from there, directed to health system area leaders. The DGSOM Office of Graduate Medical Education (GME) and UCLA Health Office of Compliance Services also offer anonymous reporting.
  • You require more local, timely attention pertaining to DGSOM-specific concerns.
  • You are unsure whether to escalate your concern to UCLA Health Compliance or UCLA Compliance/the UCLA Whistleblower Hotline, which addresses reports of actions that violate University policy, violate a state or federal law or regulation, waste money or involve gross misconduct, gross incompetence or gross inefficiency.

Anonymous reporting is an option. Additional information is available when you access the form (link below).

DO NOT use this form:

  • To report events presenting an immediate threat to life or property. If you experience a life-threatening emergency, call 911. If you require urgent assistance, please contact your supervisor, department head, Environmental Health & Safety (310-825-5689); expect responses to EHS voicemails within approximately 24 hours.

OPTION 3: Anonymous reporting for DGSOM trainees in clinical environment

For trainees with issues pertaining to the clinical care environment, questions and concerns should be addressed during huddles, escalated up your clinical chain-of-command and, from there, directed to health system area leaders. In addition, the following Offices offer anonymous reporting for trainees engaged in clinical activities:

  • The DGSOM Office of Graduate Medical Education (GME)
  • The UCLA Health Office of Compliance Services

DGSOM building occupant safety questions or concerns

Examples include concerns over building plans, signage, custodial services (disinfection), security, etc.

THREE EASY OPTIONS

OPTION 1: Call UCLA Facilities Management Trouble Call (310-825-9236) to request cleaning, restroom or hand sanitizer restocking, or to report a problem. The UCLA 311 App is another way to communicate issues and allows you to upload a photo or video.

OPTION 2: Contact the DGSOM Building Operations Team at DGSOMBuildOps@mednet.ucla.edu

Airflow/ventilation questions or concerns

TWO EASY OPTIONS

OPTION 1: Contact the EHS Industrial Hygiene Team: www.ehs.ucla.edu/eosp/industrial-hygiene or indhyg@ehs.ucla.edu

OPTION 2: Report an indoor air quality concern at www.ehs.ucla.edu/report/indoor-air-quality

Concerns or questions related to non-compliance at UCLA Health (not necessarily related to ramp-up activities at the DGSOM)

Examples include conflict of interest, privacy and information security regulations, clinical research billing, HIPAA, etc.

  • Per normal procedure, issues of non-compliance in the clinical setting should be reported to the UCLA Health Office of Compliance Services
