Program Letter of Agreement
Program Director & Faculty
The ACGME requires a Program Letter of Agreement (PLA) for each program and participating site if the assignment is for one month or longer.
Please follow these guidelines when preparing your PLA:
- The PLA templates can be found in the "Resources/Documents/Forums" section of MedHub.
- A current PLA (completely signed) must be in place before sending residents/fellows to the participating site.
- The administrative contact listed must be somebody who is functioning as a residency coordinator; cannot be the associate director or a faculty member.
- Goals and objectives of the rotation must incorporate the ACGME competencies.
- Delete all underlining and bold lettering when completing the PLA.
- After the program director has signed, submit to the Cherie Lewis in the GME Office to obtain the other UCLA signatures. Do not submit directly to the DIO.
Program Coordinator
Before submitting a new PLA, the approval request below must be submitted to the Cherie Lewis. Once approved, you will be notified by the GME office as to what action needs to be taken.
