UCLA Health is notifying 2,190 ophthalmology patients that an unencrypted, portable data-storage device containing patient information has been lost.
There is no evidence that the unencrypted USB drive has been obtained by an unauthorized individual, or that patient information has been inappropriately accessed or used.
The USB drive contained names, dates of birth, dates and types of treatment, and physician names. It did not include Social Security Numbers, credit card or banking information.
The USB drive was used in conjunction with surgical equipment to assist with procedures performed at the UCLA Health Stein Eye Institute in Westwood.
It was reported lost in July. Patient notifications began in August, following an internal review and confirmation of contact information.
Processes and surgical equipment accessories are being updated in the ophthalmology department to provide additional safeguards regarding protection of patient information.
UCLA Health takes protection of patient privacy and personal information seriously and regrets this incident.
